Care plans are personalized based on the needs of each patient and their family/ caregivers to ensure consistency in the care of the patient , to document patient needs and potential risks, and to help the patient's care team and caregivers work collaboratively toward optimal outcomes.
Services provided to the patient where they live based on their personalized plan of care for times when they are not in a medical crisis. Members of the hospice care team will visit the patient at home, whether home is their personal residence, a skilled nursing facility, an assisted living facility, or any other place they reside.
Services provided during periods of crisis for up to 24 hours per day which may include nursing, hospice aide, social worker, or personal care of volunteer services. A minimum of eight (8) hours during a 24-hour period will be available during crisis. One-half of the total hours of care provided during each 24-hour period must be provided by a registered nurse.
Services provided based on patient and family need and acceptance and in keeping with the patient/family/caregiver belief system and practice. These services include direct support from our team Chaplain or services coordinated utilizing local clergy and other individuals.
Services included for routine care on an intermittent basis when personal care is needed. These services include bathing, shampoo, range of motion activities, dry dressing changes, assisting the patient to the bathroom, use of commode or bed pan, assisting with meal preparation, feeding the patient, light housekeeping and/or linen changes.
Services and care provided to optimize the patient's comfort and dignity through appropriate treatment of secondary symptoms and aggressive management of these symptoms. Pain will be assessed at each visit and addressed as significant changes occur. Patient and family/caregiver education will focus on the use and side effects of medications, expected responses to therapy, and the importance of administering medications according to prescribed dosage and frequency.
Service provided to obtain medical equipment and supplies through designated vendors holding contracts with Amazing Grace Hospice. Amazing Grace Hospice will contract with vendors who have procedures to assure that all equipment provided is safe, clean and working as intended. Durable medical equipment vendors will educate the patient and family/caregivers of proper use of the medical equipment.
Services provided before and after the patient’s death to help facilitate a normal grieving process, to prepare family/ caregivers to function independent of hospice, and to identify a support system for the family/caregivers. These services are provided for to one (1) year following the death of the patient.
